Volunteer Application Form
This form (along with a copy of a photo ID) must be completed by all coaches, assistant coaches, directors or anyone else that has direct contact with any of our Little League youth.

Coach's Application
To be considered for a coaching assignment - this application needs to be filled out and returned by March 1st along with a completed Volunteer Application. A photo ID must accompany the Volunteer Application.

2010 Coaches Handbook and Safety Manual
This is the coach’s handbook and safety manual for East Madison Little League (EMLL). It contains information about EMLL for coaches and volunteers, including emergency contact information, names and telephone numbers of EMLL officers and directors

EMLL Scholarship Application Form
EMLL strives to provide financial assistance to as many players as possible. Changes to the Scholarship Program have been made this year so that the program can reach a broad range of needs while following established qualification guidelines.

Medical Release Form
No player can participate in practices, games or Little League activities unless the player's coach has a completed Medical Release form.

Little League Inc. Regulation 2(d) form
This form should be filled out by any player who has moved outside the EMLL boundaries and would like to continue to play at EMLL. A form needs to be on file with Little League in or

Little League Inc. Regulation 4(h)
If a person previously resided within the EMLL boundaries for 2 years while serving the league as a dedicated manager, coach or member of the EMLL Board of Directors for 2 years, his or her sons and/or daughters are eligible to play in EMLL.

EMLL - District Map
